Financial Ombudsman Claim Forms 

If the Lender fails to payback PPI (usually a decision is made after 8 weeks from when we write a letter of claim on your behalf), there will be further PPI claim forms to complete below.  We will help you complete these PPI claim forms, it is part of our PPI claim back service.

To submit a claim to the Financial Ombudsman you need to complete two forms.  If you have any queries or problems with completing the forms or downloading them, please Contact Us or call 0800 011 2757 between 9.30 am to 6.00 pm Monday to Friday for help.

If you believe you have a PPI claim, please complete our PPI Claim Form.

PPI Complaint Form

Please print off, complete and sign the complaint form (see second form below).  The Form has been saved in Microsoft Word 2003 - 2007.

PPI Questionnaire

Please print off, complete and sign the PPI Questionnaire   (see third form below).  The Form has been saved in Microsoft Word 2003 - 2007.

And Don't Forget!

When you return the Ombudsman Claim Forms please ensure you quote your reference number which is on the top right hand of our letters and emails.  Please print your answers clearly as this will help everyone.  You must send your forms back to I AM HAPPI Solicitors DO NOT send them directly to the Ombudsman.
